The template is below in a word document. You can print the template directly onto your card stock and then cut and score as shown on the template. I have added a document that contains two templates on one page. Not all printers will be able to handle the narrow margins on this document.
An alternate method to achieve the same pattern is start with a 4" x 9-1/2" piece of card stock. On the short side, score along the length at the 1" and the 3" marks. On the long side, score along the length at the 1", 3", 4", 6", 7-1/16" and 9-1/16" marks. Then you can use the word document as a guide for what to cut off.

when you put the box together, do you use tape on the side flaps?
The best adhesive to use is Sticky Strip or Terrifically Tacky Tape. Both of these tapes have a red liner. I use these for all my 3-D items and pop-up cards and it really keeps them from falling apart.

oh these boxes are adorable and i'd love to make them for the kids at school... can anyone supply the template as a PDF? i can't translate this .doc file, as i'm on a Mac platform. thanks if you can do this!
