Default Post It Note Covers--which one would be the easiest

There seems to be so many variations on making Post It Note Covers.
Since I'm not very creative, I would appreciate ones that are very easy to do and not very time consuming.

I think the simplest is the matchbook style with the staple in the bottom. I did it with a bunch of second-graders for their mothers for Mother's Day. We made a card, the post-it-note holders and a bookmark. VERY simple.

Default Post it Note holders

I've never used a Template for making any of these.
All I do is Take post it's and a piece of CS, measure it, and then cut the size i need. I make just a little longer, So I can use my deco scissors,

Default I use 2"x3" post-its

I use 2"x3" post-its from St*ples for $4.43/12 pack. I then split the 100 sheets into 50 sheets. This way I get 24 pads to use Using a 8.5"x11" piece of cardstock, I cut 5 strips going from left to right. This makes a flip up style holder (like steno pad) and enough lenght to double on the back for support. Hope this helps

When attaching Post-It-Note pads to the holder what adhesive works best?

Double sided scotch tape?
