Need to cut hole in card, and extra CS for inside front of your card big enough for your insert to spin.
Cut two inserts, Stamp and whatever. You need fishing line or other filliment.
Glue two insert with fishing line inside. Then Inside card. Take fishing line and tape so that it is taunt and centered in the hole. Take your other CS and glue over so you don't see the tape and fishing line. I hope this is ok and not clear as mud..

I used the tag punch to do this type of card.

*Punch in front right corner of base card.
*Align second layer of front of card where it should be without actually adhering & punch same punch aligning from inside of card so punches match. *Remove layer piece, decorate as needed and set aside.
*Punch your focal image twice(use small tag punch or regular tab punch and cut down).
*Adhere a length of dental floss beween your two focal images with a couple inches of floss coming off top & bottom of focal image.
*Align focal image in your punched space on base card. Adhere top & bottom of floss to base card with sticky strip.
*Adhere layer piece of base card to cover your tape.
*Admire your card and watch tag spin!
