Default fold out book template oh sort of

I saw a book some months ago, and it was about 6 inches square, I think the cover(s) was/were covered chipboard. Attached to it somehow was 4 other sheets of paper, they were probably 12 x 12. The 12 x 12 was folded somehow, maybe scored diagional, and then one corner, 6 inche size of each of the 12 x 12 sheets was attached to the 6 inch chipboard cover. So when the book was opened it was a 12 x 12, with a corner of each of the 4, 12 x 12 sheets, I think they were card stock. I think the book was tied together on one side with ribbon. I know this is a very bad discription, does anyone know what I am talking about???? Is there a template or tutorial or instructions???? What is this type of book called?????
