Where can I find the template for...

Where can I find the template for the cards I've been seeing in the gallery that 'pull' out of a little envelope thing. The egdes are rounded here is a sample in case I am not making sence (which I may not casue it's 1 am!) As always thanks for your help!

Have you tried looking in the template and technique form? Also someone posted this website a few days back and it has a ton of templates, maybe you can find what you are looking for there?
