If you search on ebay for "die cut post-it note holders", you'll see a bunch made from cardstock that are cut and ready to embellish. You could use them for a template...this link is what I am talking about.

There is an actual video tutorial on a webisode by the Terri O show. I can't post the link, but if you google the site and look in her archives you can see how to make them. Then you can print out materials and instructions. They make great gifts. I have made a lot of them and my kids have too.
I don't recall where I got it but I have a pattern for "Pretty Post-It-Notes".
It calls for one 4 x 7 1/2" cover, two pcs. of 3 1/8 x 3 1/4 for the inside, one 3/8 x 3 1/4 spine and one inside cover measuring 3 x 6 1/2. You would want your cover to be a pretty pattern and the inside can coordinate with it. It's just a matter of gluing the spine and 2 pcs on the inside leaving a slight gap for folding. Once you have glued those down you would adhere the inside cover to hide the folds. When everything is dry you can glue the last page of your Post It note to the back cover and you're done. I made 75 for my daughters wedding and everyone loved them. I have the template but don't know how to attach it to this note.
I have made quite a few from Angie's tutorial. They come out very nice and are quite sturdy. I want to do some Halloween ones, I just haven't gotten to it yet.
Thanks for sharing these great ideas! I think that a little gift bag with a personalized pen, a fancy post-it note holder and a baggie of chocolates would make an awesome teacher Christmas gift!
I just made several and sold them at my craft show Saturday. I used pieces from the DCWV Chipboard Stacks and put them together with my Bind-It-All. (and decorated!) Sorry, but the pix aren't up on my blog yet. :-(
It's interesting because one of the gals said she was glad to see me doing them that way this year. Last year her friend got her one of mine out of cardstock, and she said these were "better because they'll hold up longer."
They were REALLY easy the way I did them this year!!!

Oh, and post-it-note books in general are always a hit. I used to make the book kind (the ones referenced above that use a 7-1/2" of cardstock to make the jacket) for craftfairs and always sold out. Also did sets of these paired with junior legal pad covers that matched. Once you get in the groove, they work up so quickly, they make a perfect high-volume project.
This year I loved the little ones with the pen on the side, so I just purchased one of the ones in $1 bin at Michael's and took it apart to make my own pattern. Lots of times if you can't find a pattern to download, it's quick and easy enough to take something apart and "reverse engineer" it, lol!
The Blue one is the first one I made. I added the Ribbon and Keychain ring. And attached the ribbon with Mono Multi Glue (green & white bottle) and then added a keychain ring with a brad.
Here are two of mine using discounted Post-It style notepads that I got for 50� at Michael's. The pads were 3" square and had black & white patterns on the edges.
