Default post-it-note holder

I was going to do some of these for my kid's teachers along with a few other things. I'm sure it's quite simple but was wondering if there were instructions/measurements anywhere? I used the search function but didn't come up with anything. Is it a paper just folded in 3rds and the top is cut short? Is it really that simple?
