Please Point me in the right direction for the onsie card how to

I have promised to make some baby shower cards for a lady I work with...mind you I have not stamped in well over three years! I went hunting for ideas and saw the onsie card, but couldn't find the instructions anywhere in my searches.

Would someone be so kind as to point me in the right direction? Is there a tutorial somewhere or a template? The lady I'm doing this for saw the onsie card and fell in love with it, so looks like that is what I'm gonna be doing.
