Default Box to hold cards

I am looking for a template for a little box, a standing up kind of one, that will hold cards in it. I saw one somewhere and have no clue how to make it!!!

I will try attaching a jpeg file with some instructions. Sorry I don't have a photo as an example, but I tried this template out and it worked fine.

If you find something better in the gallery, great! But in case you don't, at least you have something to start with.

Here is one that is made with one sheet of 12 x 12 paper. It can easily hold 6 cards:
