Hi! I can't find the template to give credit (once you make a couple of these it is very simple!), but found it through the gallery area of "anything but a card", then "5 in 1 template." It took me to a tutorial at a great website I think is called "stamp time somewhere." I am sorry I can't remember the details! If you don't find it, PM me and hopefully I will have found my info by then!
