This is the project I did for the SoCal Mini Mania on September 30, 2006. I made up the program in Excel, and then cut them apart. Originally I wanted to do this as a 1/2 page project, but I thought that this smaller version would be cuter, so I cut it down.

O.kay - this is an AWESOME idea! I'm am always forgetting user name, ID and passwords for all the different sites. This is a great idea! Thanks for sharing.
