My DH's coworker had a baby boy. They wanted a card to pass around the office for everyone to sign. And they wanted to be able to include a gift card, so there is a slot in the one panel.

The card is actually made of maybe 4 pieces of paper adhered together with red line tape or whatever it is called. I think I had two pieces that were 12" x 5 1/4" - folded at 4" and 8". Then I think I had two other pieces that were 8" by 5 1/4". You kind of have to fuss with it because you need the correct number of folds to make the two ends pieces "face up" when it is open. (if that make sense). I might be be remembering what I used correctly. But piecing the sections together also allowed me to put the slot in the one panel for the gift card. Just be sure to only tape those pieces together around the edges, so that the gift card can slide down in.
