There are three sizes of rectangular curvy label or note shaped templates: LARGE, MEDIUM, & SMALL. But you can manipulate them with software or printer settings.
My favourite way to use them is to print them on the back of DP or cardstock & cut them (that way you don't see the lines. But I've also used them to print onto cardstock & cut with an exacto knife (hobby blade/craft knife) and then you can use both the frame and the inside piece.

If you'd like to make your own shapes, here's how I made them:
1. Folding a piece of paper in 4
2. Cutting a corner in a fancy shape
3. Unfolding & tracing onto the text cardstock
4. scan
5. Print multiple copies of the scanned shape
and 6. scan it again
