This was fun to try. Saw it on Susan Elise Morton's blog:StampingPurrfection using this dsp and found the deets using diff. dsp. on Laura Borchert's (Mar.28/2017) blog:laurasstamppad.com. dsp is 4x5-1/4"; white panel is 2-1/4"x 5-1/4". on dsp, the middle is at 2-5/8", then cut in 2", and fold back, adhering with some snail to make them lie flat. You can add any embellishment you like. I used ribbon. Laura used a punched flower.
This is for Cathy's challenge to focus on the sentiment, although I am sure you are focusing on those folded back flaps, lol. ~thanks for looking~

MARVELOUS collar fold card...your paper pattern paper is quite a wonderful choice opening to display such a fun bright inside! Great sentiment placement in the open collar area. Love, love your ribbon and twine embellishment...such a perfect finishing!
