I use a pair of needlenose pliers to undo the ends of the spiral. I then just roll it out and tape my cardstock to the notebook cover. Then I take my 1/8" hole punch and punch the holes using the existing cover as a guide. Then I just roll the spiral back through and bend the ends back. It's pretty easy. I like to call it my Survivor night project since it's something you can do without thinking.

I'm new to this site and I'm having so much fun exploring all the different and wonderful suggestions given. I love the notebooks above and will be using this idea for gifts this Christmas. Thanks so much for sharing.
