I have a piece of fishing line in my tool kit - make a loop and put the open ends through the button holes, then feed the ribbon into the fishline loop and pull through. Works like a needle threader but you can pull a bit harder
Now, that's a cool  idea for big ribbon in a small button!
I also sometimes just use a small "thread" of cardstock through the holes to make the button look like it's sewn on and finish off the holes. I think it looks unfinished to have "naked" holes. Just thread them through to the back and secure them with the glue dot used to hold the button to the card.
I use a Big Eye needle - for jewelry making but works like a charm for most purposes when scrapping. It's soft so you can't make holes with it, but I think that's the only limitation I've found. 4" long and has a 3" hole...
I use dental floss threaders too - you can find them in the toothpaste aisle at Target. Here's a link from a while back where JulieHRR showed what they look like if you've never seen them: http://papertrufflez.typepad.com/pap...lly_paper
