Accordion card instructions?

Actually it is different from the waterfall card. This one has a"spine" folded into an accordion style. The squares of cs are sort of placed randomly.

I don't know the name of this card, but it is an accordion type card with stamped squares attatched at different levels on the accordion. Does anyone know the name and have the instructions?
