Looks like you cut your paper 4.25 x 11, score at 5.5 and 8.25 for the accordion folded part. Then just make your image panel and adhere it to the mountain fold, tuck it under at the top of the card when finished.
I've never seen one before, but that's what it looks like to me.

Here's how I did it, thanks for asking! Score your 4&1/4 X 11 piece at 2&3/4 and at 5&1/2. The white piece is 5&1/4 X 4. The pattern piece on the bottom is 4 X 2&1/2. Attach them. Circle is 1&1/2", but we used a larger one in class, so whatever you have. Hard to punch through layers for the eyelet, so I punched the circle, then got it where I wanted it and punched the card. Attach your stamped piece just at the bottom of the pattern paper, and so that the top of it is just a little below the center score when laying flat open. I hope this isn't confusing. We tied ribbon across the bottom of the stamped piece and card in class, but it was smaller than the one in the picture, so I left it off this time. Hope this helps!

That's a variation of what I posted above.....the distance between the scores is exactly the same. For proper accordion scored lines, which will create the best folding, it's best to have a mountain and a valley score. Make sure the mountain is on the 5-1/2" and the valley on the 2-3/4" or 8-1/4" (both work, just not both of them ).
